Asociación entre ganancia de peso gestacional y peso al nacer en la díada madre - recién nacido en un hospital de referencia de Asunción y Coronel Oviedo

Introducción: La ganancia de peso gestacional (GPG) es un determinante del crecimiento fetal, independientemente del IMC pregestacional.
Objetivo: Evaluar la tendencia entre la ganancia de peso gestacional total y semanal y el peso de nacimiento en una cohorte de gestantes paraguayas atendidas en hospitales de referencia.
Materiales y métodos: estudio observacional analítico prospectivo con 100 gestantes ≥18 años, embarazo único, seguidas en tres controles prenatales hasta el parto. La GPG total se calculó como la diferencia del peso entre tercer y primer control, la ganancia semanal como la ganancia total dividida semanas transcurridas; ambas se clasificaron en insuficiente, adecuada y excesiva. El peso de nacimiento se analizó como variable continua y categorica.
Resultados: La edad materna promedio fue 28,5±6,2 años. La GPG total media fue 8,3±4,6 kg y semanal 0,2±0,1kg. 90% de los recién nacidos fueron de término y 87% presentó peso adecuado, promedio 3342,1±484,6g. El peso al nacer mostró un incremento progresivo según la GPG total (3278,3g, 3292,6g y 3570,0g para ganancia insuficiente, adecuada y excesiva, respectivamente), con diferencia significativa entre ganancia adecuada y excesiva (p=0,044). No se observaron diferencias en GPG semanal. La macrosomía fue más frecuente en el grupo con ganancia excesiva y el análisis lineal evidenció tendencia positiva entre mayor GPG total y mayor peso al nacer (p=0,019).
Conclusión: La GPG total mostró una tendencia positiva con el peso de nacimiento, destacando la importancia de monitorizar la ganancia ponderal durante el embarazo y generar evidencia local para optimizar recomendaciones de control prenatal en Paraguay.
